If you're a new client, please download and complete the " New Client Intake & Disclosure " form (below), and bring it with you to your first session. Be sure to read it carefully, and sign at the bottom of page 3. You may want to print an extra copy of pages 1-3, just for your records. **These forms are being updated, and may include old contact information. Do not worry about this, thanks :)
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), download and complete the " Release of Information " form (below) to authorize release of psychotherapy information:
